Commonwealth Carelink South East Sydney aims at
promoting greater collaboration in care planning for older
people, younger people with disabilities, through the
establishment of crucial links between the health and
community care sectors. A comprehensive database
of information on services available in the HACC South
East Sydney region, enables staff to provide up-to-date,
accurate information either in person or by phone.
The Centre provides a central source of information that
links health professionals, general practitioners and other community service providers, carers and individuals with agencies providing care and support in the region
